Join Baylor Scott & White Health, Parkland Health and Hospital System, Texas Health Resources, and Methodist Health System for an opportunity to create partnerships that meet the clinical and social needs of patients in our communities.

Why Attend?

This full day symposium brings together community agencies, faith-based organizations, community health workers, nurses, social workers and others who help refer and connect patients to community resources across Collin, Dallas, Denton and Tarrant counties. The goal is to bring everyone addressing the health-related social needs of the underserved together to discover ways to better serve individuals and their families.


  • Build connections between the people who refer and those who receive referrals.
  • Synergize, synchronize and strategize to build networks to meet the clinical and social needs of patients in our communities.
  • Interact through case studies, round-table sessions and participate with community service providers in a brainstorming exercise.
