Hospice and Home Health Care

We offer expert care in the convenience of your patient’s home while providing you ease of access through our electronic medical record (EMR).

Hospice and home care at Baylor Scott & White Health in Central Texas provide an array of home-based services for patients of all ages with acute rehabilitation or acute palliative needs.

Our services are overseen by a medical director and are coordinated through our electronic medical record (EMR), which enables ease of multidisciplinary coordination.

Hospice Care Services

Our hospice care services are designed for adults with terminal illness. Our multidisciplinary team places emphasis on providing care that meets the physical, emotional and spiritual needs of your patient and the patient’s family.

Hospice Care at Baylor Scott & White Health in Central Texas is a Texas Licensed Hospice, Medicare-Certified, Medicaid-Certified, Non-profit 501(c)(3) Organization. We are accredited by The Joint Commission.

Eligibility

Conditions that are generally eligible for hospice care include:

  • Adult failure to thrive
  • Dementia
  • Chronic degenerative neurologic disease
  • Heart disease
  • HIV/AIDS
  • Huntington’s disease
  • Liver disease
  • Parkinson’s disease
  • Pulmonary disease
  • Renal failure
  • Stroke
  • Coma

Eligible patients must have a terminal condition with estimated life expectancy of six months or less if the illness runs its normal course. A family member or caretaker must be willing to assume 24-hour responsibility of care when safety becomes an issue in being home alone.

Home Health Care Services

Our home health care department in Central Texas provides care to patients who are homebound and have an acute medical need or require skilled nursing or therapy in-home services. Homebound patients who receive home care services typically fall into a wide variety of categories. They may have an acute medical need following surgery, a new onset of a disease or an exacerbation of a current condition.

Our home health care services include:

Nursing care

  • Observation and evaluation of patient’s condition
  • Education about their medications
  • Wound care
  • Foley catheter insertion and care
  • Health education on disease processes
  • Injections
  • Lab draws
  • Wound-Vac
  • Home safety evaluation

Occupational therapy

  • Home safety inspections to improve function or prevent injury
  • Teaching patients how to perform daily tasks (such as cooking and personal care) in consideration of the patient’s current ability level
  • Evaluation of need for adaptive equipment. Adaptive equipment is a device to assist with completing activities of daily living, like reachers and sock aides
  • Training patients how to use adaptive equipment if needed

Physical therapy

  • Therapeutic exercise
  • Functional training

Speech therapy

  • Assessment, diagnosis and treatment of disorders related to:
    • Speech language
    • Cognitive-communication
    • Voice
    • Swallowing
    • Fluency

Infusion therapy

  • Administration of intravenous (IV) medication for:
    • Bone/joint infections
    • Bacteremia
    • Cellulitis
    • Pneumonia
    • Cystic fibrosis exacerbations
    • Short bowel syndrome
  • Family education
  • Social work services
