Geriatric Medicine

Geriatricians are specifically trained to care for the complex needs of older adults in order to reduce hospital readmission rates.

The geriatric medicine department of Baylor Scott & White Clinics in Central Texas offers primary and specialized post-acute care for rehabilitation to skilled nursing facilities, assisted livings and the community at large. We work within the community to promote health and wellness by conducting a comprehensive geriatric assessment. Our efforts have led to better outcomes for our patients including reduction in pressure sores, use of antipsychotic medications, and hospital readmission rates.

We provide multidisciplinary care teams, electronic medical record services, coordinated health care services and 24/7 on-call staff for the facilities we support.
