Request for Access to Epiccare link for an external organization
EpicCare Link is a web-based application connecting Baylor Scott & White Health community based external providers and staff via secure access to select patient information in the Baylor Scott & White Electronic Medical Record (EMR).
Providing a full SSN ensures that duplicate user accounts are not created in the BSW system and allows for a more streamlined activation and help desk experience. Alternatively, last 4 digits of SSN will be accepted. In addition to completing the information below, all applicants need to 1) Sign the "Electronic medical record access agreement for non-Baylor Scott & White Health users" and 2) Review the "Baylor Scott & White's EpicCare Link HIPAA Training." One person at each clinic must be assigned as the site admininistrator. This user is the primary designated EpicCare Link contact at your location and has authority to deactivate users that leave your clinic. The access process usually takes 1-2 weeks for your access to be granted for EpicCare Link.
